Before your Appointment

If your appointment is scheduled a few weeks in advance, we suggest you jot down your questions and concerns so you can share them with our staff.

Please plan on arriving 15-30 minutes before your scheduled appointment time. This will give you time to register and complete any paperwork that is needed. For your convenience, registration forms are available online.

Children

Children are welcome in our office, but please remember they have a short attention span and can quickly become bored “watching the baby.” We ask that you bring another adult to watch younger children during your appointment. If you are unable to make other arrangements, please call to reschedule your appointment.
